2025-26 Intercollegiate Student-Athlete Medical Forms

Sacred Heart University Athletic Training utilizes an online system to track injuries as well as organize medical forms.  ALL student-athletes (incoming and returners) must log in to the online portal to update their medical health information and review items that are required on an annual basis.

The mission of the Athletic Training Department at Sacred Heart University is to deliver the highest quality athletic training and sports medicine services to our student-athletes. Our Board of Certification Certified Athletic Trainers are licensed by the state of Connecticut as healthcare professionals. Athletic Trainers (AT's) are healthcare professionals who collaborate with physicians to optimize the activity and participation of our student-athletes. Athletic training includes the prevention; clinical evaluation and diagnosis; immediate care of athletic injuries as well as treatment, rehabilitation and reconditioning of athletic injuries. Athletic training is recognized by the American Medical Association (AMA) as a healthcare profession.

The Sacred Heart University athletic training staff is comprised of eight full-time and five graduate-assistant/intern athletic trainers who work alongside Orthopaedic Specialty Group. Their practice also provides access orthopedic surgeons, chiropractors and a physiatrist with varied specialties. Our sports medicine team also includes general physicians, dentists, general surgeons, neurologists, internists, dermatologists and a radiology office that serve as referral services for our department. In addition we work closely with our university Health Services department in order to provide well rounded care for our student-athletes.

The overall health and well-being of the student-athletes are enhanced through our CAATE-accredited undergraduate athletic training education program. Athletic Training Education Program undergraduate students complete clinical experience assignments with teams on campus as well as at many other off-campus locations.

The athletic training staff members serve as adjunct clinical professors as well as Approved Clinical Instructors in the undergraduate athletic training education program.
